Background With the advent of precision medicine, pharmacogenomics data is becoming increasingly critical to patient care. These data describe the relationship between a particular variant in the genome and the response to a drug by the patient. As utilizing this kind of data becomes more integral to medical treatment decisions To achieve integration of pharmacogenomics into clinical practice via MTM, the pharmacy profession must define a process for the application of pharmacogenomic data into pharmacy clinical practice that is aligned with MTM service delivery, develop a viable business model for these practices, and encourage and direct the development of HIT

Despite scientific and clinical advances in the field of pharmacogenomics (PGx), application into routine care remains limited. Opportunely, several implementation studies and programs have been initiated over recent years. This article presents an overview of these studies and identifies current research gaps. To achieve integration of pharmacogenomics into clinical practice via MTM, the pharmacy profession must define a process for the application of pharmacogenomic data into pharmacy clinical practice that is aligned with MTM service delivery, develop a viable business model for these practices, and encourage and direct the development of HIT Pharmacogenomics (sometimes called pharmacogenetics) is a field of research that studies how a person’s genes affect how he or she responds to medications. Its long-term goal is to help doctors select the drugs and doses best suited for each person. It is part of the field of precision medicine, which aims to treat each patient individually. Thus, while some consider current PGx tests as having inadequate value for clinical application, tests with comparable diagnostic test criteria are currently being used in patient care. The need for well-defined PGx test criteria has been previously discussed [20,21]. While pharmacogenomics seem to offer the ability to improve patient care (and hence therapeutic outcomes), the clinical adoption of pharmacogenomic interventions has been slow. While many of the drugs commonly prescribed in general practice (eg warfarin, fluoxetine) are influenced by genetic variation, there is currently no clear recommendation in Australia about the use of pharmacogenomic This has been shown to reduce the risk of adverse events and improve patient health-care outcomes through targeted therapies and dosing. As the field of pharmacogenomics rapidly evolves, the role of pharmacists in the education, implementation, and research applications of pharmacogenomics is becoming increasingly recognized. With the initiatives by the National Institutes of Health and the US Food and Drug Administration, pharmacogenomics is transitioning from the laboratory to patient care. Nearly 200 drug products now contain pharmacogenomic information as part of their labeling; many of these products are commonly used in the pediatric population.
